Program updates 

Keep us up to date on what’s going on with your project! ALL notable programmatic and/or administrative changes taking place at your organization in relation to CHAI (including staffing, budget, implementation, etc.) must be reported to CCHA as soon as you are aware of the change here: https://forms.office.com/r/9QEcCvgw8s 

Monthly reporting 

There are two ways to report your monthly data 

1. Enter all your data into Redcap

In order to get you access to the REDCap database to enter data, complete this form for any team member(s) who will need to be able to see their organization’s data in REDCap

2. Submit a monthly spreadsheet

Use the link below to upload all of your monthly reports

Please do NOT email reports to CARE and / or CCHA moving forward unless you are requesting review in advance of the deadline.

Please update your information by the 15th of each month.

Forms to report each month 


There are three forms that we ask you to complete for each participant: 

1. Participant information form

The purpose of this form is to capture data about the participant that will stay consistent over time.  

You only have to complete this form once for each participant, at their “intake” into the CHAI program

2. Encounter form

The purpose of this form is to capture data about CHW activities with each participant over time.

Complete this form each time a CHW has an “Encounter” with a participant. If a CHW has multiple touchpoints within a single day, they can be entered as a single encounter.   

3. Arizona Self-Sufficiency Matrix (ASSM) form

The purpose of this form is to capture information about participants’ social determinants of health, which may change over time. 

Complete this form within the first few encounters with a new participant. Then complete an additional form for the participant at least every six months OR every time there’s a “substantive” change – such when a need is met, a referral completed, or a major life change.
